In the space provided below, please insert a general account of the company’s activities in the financial year to which the report relates, including a description of how they have benefited the community. Crossway Alliance CIC runs a health promotion and health education service with emphasis on healthy lifestyle and behaviour change to improve quality of life, health outcomes and the prevention of illness. We commenced service in August 2023. During the financial year 2023-2024, Crossway Alliance activities have benefited the community in the following ways:Local residents (particularly those newly diagnosed and those living with health conditions) have been able to access our service conveniently, either directly or through our network of partners at various venues where we carry out our community activities. The service has provided opportunities for people to come together to discuss what matters to them, connect with each other and to the resources within the community for optimal health and wellbeing.The number of established groups and organisations we work with have increased in numbers. Implying that we have been able to reach out to the wider community than expected. Our service has offered people in the community the necessary information on how to take control of their health by adopting positive lifestyle and behaviour change that ultimately reduces their chances of developing preventable illnesses. Through our service we have been able to engage with the hard-to-reach groups in the community and encouraging their participation in community health and well-being interventions.Over 80 households have been supplied with blood pressure monitors through the company’s healthy lifestyle service.
The company’s stake holders are residents of and visitors to Heywood and Rochdale community and other VCSFE’s in the community. The directors and members have lived in the community for a number of years and developed the business model for the service based on our experience as a healthcare professional working in the community and talking to other residents. We hold meetings with groups and organisations asking everyone about their health needs to identify barriers. This has helped us to build a profile of local VCSFE’s and to better understand what community members needs and what we can do to meet these needs effectively. Our research has enabled us to identify local health needs, the health concerns of residents and helped them to navigate the resources within the community for their optimum health and well-being. We have been able to adapt our services to suit the needs of the residents through our health education and health promotion workshops. We encourage feedback from workshop participants and have made available suggestions box for organisations to provide us with details of health issues they would like us to address
