Company activities and impact

SMART Cranleigh (SC) covers Cranleigh Ewhurst and surrounding hamlets. It improves community wellbeing by facilitating active relationships with providers of services, signposting and co-ordinating their accessibility for the community. During the financial year it’s activities have included: - - Provide ongoing coaching and support for local dementia and carer group leaders. - To broaden impact, we are working towards developing Community Connectors linked to the Directory, including large public meeting to raise awareness. - We are undertaking an updated community directory to help people navigate social activities to join for all ages and access the health and support they need via a central website working with Surrey County Council -We are working closely with the Surrey Heartlands (ICB) team to support integration of health and wellbeing needs in Cranleigh and the surrounding area and to help ensure that the statutory authorities hear the local voice and improve partnership working. A number of regular coordination meetings have continued within Primary Care and the Health Alliance - Set up Cranleigh Villages Group meeting to develop health creation locally working in partnership – this is ongoing including public meetings to engage stakeholders. - Continuing support for a walking group for people with dementia and their families.

Consultation with stakeholders

Continuing the working with the community stakeholders to provide a local directory of community activities including joining information held on different websites into a sustainable mainstream website. Collaborating with Chamber of Commerce, Arts Centre, Leisure Centre, day centre and local churches to develop initiatives that address need and encourage collaborative partnership working across the community e.g. local Directory, Dementia groups and Welcome Cafe. Initiating dialogue and SWOT analysis with surrounding Parish Councils regarding Health and Wellbeing needs and assets within their community stakeholders.
