During the financial year, SW Health and Wellbeing CIC has continued to make a meaningful and measurable difference within the community, with a particular emphasis on supporting young people aged 14–25 who have Special Educational Needs and Disabilities (SEND). Our programmes are designed to develop essential life skills in a relaxed, engaging, and supportive environment, helping participants build confidence, independence, and overall wellbeing. A central focus of our work has been the delivery of inclusive, person-centred activities that promote social interaction, physical activity, and mental wellbeing. Through structured workshops and practical learning opportunities, young people have developed key competencies including communication, self-care, and everyday living skills. All activities have been carefully tailored to ensure accessibility and enjoyment for every participant, regardless of their individual needs. To maximise impact, we have continued to strengthen partnerships with local authorities, educational institutions, and community organisations. This collaborative approach has enabled us to align our services with existing provision, helping to create a cohesive support network and avoid duplication. Working alongside local authority-commissioned programmes has further extended our reach and enhanced the quality of support available to young people with SEND. Monitoring and evaluation have remained integral to our approach. We have actively gathered feedback from participants, families, and stakeholders, enabling us to respond to community needs and refine our delivery. This commitment to continuous improvement ensures that our services remain relevant, effective, and of the highest possible quality. The outcomes of our work have been wide-ranging and impactful. Young people have demonstrated increased self-esteem, improved physical health, and stronger social connections. Families and carers have expressed greater reassurance and support, knowing their loved ones are engaged in meaningful, safe, and enriching activities. By equipping young people with the skills needed for greater independence, we are contributing to a more inclusive, empowered society. Looking forward, SW Health and Wellbeing CIC remains dedicated to deepening our impact and extending our reach. We will continue to design and deliver innovative, responsive programmes that support young people with SEND to thrive, fulfil their potential, and lead enriched, independent lives.
