General Account of Bow Health Network Primary Care Network’s Activities During the financial year, Bow Health Network Primary Care Network (PCN) delivered a range of initiatives aimed at strengthening primary care services and improving the health and wellbeing of the local community. The network focused on collaborative working, prevention, accessibility, and community engagement to ensure high-quality, patient-centred care. Activities and Initiatives Coordinated and Integrated Care Bow Health Network PCN prioritised coordinated care by bringing together general practitioners, nurses, and allied health professionals. This multidisciplinary approach, delivered through regular practice multidisciplinary team (MDT) meetings, supported the effective management of complex patient needs and contributed to improved clinical outcomes. Health Promotion Programmes – Communities Keeping Well The network delivered targeted health promotion activities addressing key local health challenges, including diabetes, cardiovascular disease, and mental health. These initiatives included health education workshops, screening activities, and the distribution of educational materials to encourage prevention, early intervention, and healthier lifestyles. Enhanced Access Services To improve access to care, Bow Health Network PCN extended clinic opening hours and introduced digital consultation services. These developments increased flexibility and accessibility, particularly benefiting working patients and individuals with mobility or access challenges. Community Outreach and Engagement The PCN organised regular community events and health fairs to engage directly with local residents. These events provided opportunities to share health information, offer opportunistic advice and consultations, and strengthen trust and relationships between healthcare professionals and the community. Vaccination Programmes Bow Health Network PCN worked in partnership with community pharmacists to deliver the Spring COVID-19 vaccination programme and led the delivery of the Autumn COVID-19 vaccination programme, supporting public health protection across the local population. Benefits to the Community Improved Health Outcomes Preventive care initiatives and health education activities supported better management of long-term conditions and helped reduce pressure on urgent and emergency care services. Increased Accessibility to Services Extended opening hours and digital consultations ensured a wider range of patients could access timely and appropriate healthcare. Stronger Community Engagement Outreach activities strengthened relationships between healthcare providers and residents, encouraging proactive engagement with health services and supporting long-term community wellbeing.
The stakeholders of Bow Health Network include residents, patients, and staff within our locality who rely on or contribute to our healthcare services. We actively consult with these stakeholders through a variety of methods to ensure their voices are heard and their needs are addressed effectively. Methods of Consultation: Patient and Staff Forums: Regular forums are held to provide stakeholders with opportunities to share their experiences, concerns, and suggestions. Patient Engagement Days: These events are designed to engage directly with patients and the public, offering updates on our activities and seeking their feedback. Community Events: BHN organises public engagement events throughout the year to connect with the broader community, present updates on our initiatives, and gather input on service delivery improvements.
