The company continues to be managed by a Primary Care Network, a collection of GP Practices that work together as one entity to manage and distribute NHS funding for the delivery of services that impact all members directly. Funding is still set aside by our Primary Care Network for the primary focus of projects to support community-based care and with this it was determined that a company should be set up to manage and administer the projects directly. Within the Primary Care Network, we have an extensive Social Prescribing and Mental Health Team for adults and children and our area of cover means our patients are from all walks of life and need different community-based engagement. Especially noting many do not like to visit a GP. The company encourages health & equality for all and has projects in Wharfedale, Airedale & Craven. The company offers health clinics in the community, BP checks, blood sugar checks, vaccinations, health & wellbeing, healthy eating, and fitness classes. Specifically, the company has fitness classes specifically designed for the following categories of patients: IE chair bound, limited mobility, a medical condition including stroke patients, Parkinson’s, or multiple sclerosis. In addition, the company operates: - Women’s Health sessions. - Singing for Health - The availability of Blood Pressure machines to local areas. - A project called Eat, Move be Happy which is a community project to help patients gain confidence. This involves weight management, support groups, exercise classes, healthy eating guidance and much more. - A new project & Support groups based around menopause has been rolled out. - Health Check program in and out of a surgery setting. - Autism workshops - Stroke & Osteoporosis support groups - Cancer care and screening awareness - Children’s Special Needs Workshops - Pre-Diabetes events - Dedicated Female only Events Keighley - Gr8minds Children’s social prescribing events - Men’s Health Events
The company’s stakeholders are made up of various groups in the local community who support the projects being undertaken and help make decisions on how the money is best spent in the community. Primarily these are made up of patients, leads at each surgery within the Primary Care Network and specialists in each area. Regular informal consultation takes place with all these stakeholders to ensure delivery of projects that are impacting the areas of the community that need it and to which funding is received for.
