Provide additional support for local dementia groups and ensure they are joining up and complementary. To broaden impact, we have developed Community Connectors linked to Directory and implemented pilot training ready for roll out. We are undertaking an updated community directory to help people navigate social activities to join for all ages and access the health and support together with local parish councils. Set up Cranleigh Villages Group meeting to develop health creation locally working in partnership, this is ongoing Establishing a walking group for people with dementia and their families. Analysis of community health and wellbeing survey. Implementation of community questionnaire to better understand community transport needs across villages
Consultation with 80 stakeholders in Jan 2026. May-June 2026 consultations with 3 local Parish Councils. Community Transport consultation with 4 local villages July 2026. Ongoing consultations with NHS and Social Care agencies towards neighbourhood heath care and prevention.
