Tonsmor Doctors CIC is a not for profit community interest company limited by guarantee and without shareholders. The Company holds no bank accounts, remains dormant and did not trade during the year to 30 November 2025. Achievement of Tonsmor’s objectives, which are to help advance the design of technology and promote its adoption to deliver healthcare free at the point of care to NHS patients and make the job of the doctors and healthcare related professionals that serve those patients less stressful, took a leap forward in in terms of impact in September 2025 when the then Prime Minister Sir Kier Starmer and Health Secretary Wes Streeting each announced the creation of NHS Online. NHS Online will, from 2027, offer patients the option of an online consultation to obtain advice on managing their condition, access digital prescriptions from their local pharmacy, book tests, treatments and procedures through the NHS App at locations close to home or work. Tonsmor Doctors from Dr Morton’s pioneered this approach and has the evidence to show how it can work in a primary care, intermediate care and secondary care settings without any increase in NHS budget. NHS Online is expected to deliver up to 8.5 million appointments and assessments in this way in its first 3 years. The UK remains short of some 6,000 doctors in a primary care setting making it difficult for patients to access appointments at their NHS GP surgery and leading to less appropriate and more expensive use of secondary care facilities. Doctor shortages tend to be greatest in areas of multiple deprivation, particularly areas of rural, coastal or industrial heritage of greatest patient need, in what is defined by doctors as the ‘inverse care law’. Pending launch of the NHS Digital Hospital in 2027, the NHS Wait List remains stubbornly high, at 7.3 million. Tonsmor evidence from having provided online women’s health hubs shows that early intervention online in an intermediate care setting avoids the need join the NHS Wait List in the first place, There are many doctors that want to help with pockets of capacity, for example amongst gynaecologists to address women’s health needs. There are now many junior doctors who have completed their training only to find a shortage of substantive consultant posts in the areas in which they live. The doctors needed are just not in the right place. Those with families can’t easily be relocated to areas of greatest need. But if the right technology is applied they can serve patients and help tackle inequality by being brought online. The Tonsmor concept enables doctors from parts of the country with the capacity to help, to work online and take care to the parts of the country where it is needed most. By promoting flexible working online from home using technology, Tonsmor is able to facilitate a better life for doctors and take some of the stress out of the NHS. Flexible working provides a way for doctors with child caring responsibilities or for those close to retirement to continue to reach patients rather than risk losing them from the NHS work force. Further impetus is expected at the time of writing from the new Prime Minister and his new Health Secretary Yvette Cooper. Andy Burnham is a former Health Secretary himself and recent Mayor of Manchester which became the first Marmot City Region to address the wider determinants of health inequality. Addressing health inequalities is a priority if “good growth in every postcode, and hope in every heart” is to be achieved.
The Company’s main stakeholders are patients and doctors. The Director has consulted regularly with both over many years to obtain feedback on the design of the best technology and continues to seek feedback via the Tonsmor Doctor web site contact form. As a result of this prior consultation process the Tonsmor Doctor web site is able to summarise the four main bottlenecks to the patient pathway and in 2024-25 proposed technology solutions as a blue print for the NHS. Tonsmor Doctors can not claim to be the sole source of ideas for NHS Online or the Digital Hospital. There are many online doctor organisations like Dr Morton’s, but by actually implementing the ideas to gain evidence in primary, intermediate and secondary care, the fears of patients and doctors who were once reticent to adopt new ways of receiving and providing care, can now be allayed. Online consultations are now an accepted way to provide good medical care. It was heartening to read the results of an NHS commissioned poll in March 2026 which reported that almost two thirds of NHS consultants are keen to work for the NHS’s online hospital when the service launches next year. Six in ten consultants (60%) said they would be interested in working for NHS Online alongside their current NHS roles – many more than will be needed to run the service. The survey of nearly 300 consultants and specialist doctors also found that nearly half (48%) would be willing to offer at least four hours a week of their time, with the opportunity to care for patients innovatively, work flexibly and improve patient experience among their top reasons for wanting to sign up.
